My father needs to get a new/used wheelchair accessible van for my mother. She has been in a wheelchair for over 20 years. I need to know if my father should by one with a lift already in it or buy one that needs one put in to it for tax purposes to claim the medical expenses deduction?

You could claim the medical deduction for both.  However, you may have a stronger argument if you buy the van with a lift already in it because its main use is for medical.   If you just install the lift the IRS may argue that you only get the expense of installing the lift.   I hope this answers your question.
