What do I do if I lose my W-2 Form and my employer doesn't have an extra copy?

You should make every attempt to obtain a duplicate copy from your employer.  If you do not receive it by February 14th, you can contact the IRS, and they will contact the employer for you to request the missing form.

 

However, you can always use your last paystub of the calendar year to estimate your income and withholdings.  You would also fill out form 4852, "Substitute for W-2," and attach this to your Form 1040 (if paper filing).
