I currently send my children to a child-care center. What information do I need request from the facility to be able to claim a tax credit for child-care expenses?

You need to request certain pertinent information from the child care provider to be able to claim a deduction. You need to report their name, address and employer identification number on form 2441 unless they are a non-profit organization.
